Weight Loss Therapy
Monthly package includes 1 month supply of GLP-1 with needles and syringes. GLP1's can include Semaglutide or Tirzepatide. Elevate Medical will determine which option is best for you based on your medical history. This plan will renew each month on the same day you made your first payment. You will receive a new shipment each month containing your GLP-1 along with supplies. Prices may vary depending on dosing.

1 Patient Registration

2 Receive Instructions
Receive an e-mail within 24 hours containing documents to get you started.
3 Go to LabCorp
Bring your lab requisition into any Lab Corp location near you to be completed.

4 Physical Exam
Print out the physical exam form and have it completed by a licensed medical professional.
5 E-sign
Waiver
Click the link in your e-mail that will take you to our liability waiver. E-sign the document and submit the form.
6 Select Treatment Plan
Elevate Medical will call to review your labs and discuss a treatment plan that will benefit you.
