We are proud to offer our services nationwide for those not located in Southwest Florida.The following steps outline the process for our out-of-town patients.
1 Patient Registration
Please fill out our secure & private patient registration form.
2 Receive Instructions
Receive an e-mail within 24 hours containing documents to get you started.
3 Go to LabCorp
Bring your lab requisition into any Lab Corp location near you to be completed.
Prepaid cost is $99
4 Physical Exam
Print out the physical exam form and have it completed by a licensed medical professional.
5 E-sign
Waiver
Click the link in your e-mail that will take you to our liability waiver. E-sign the document and submit the form.
6 Select Treatment Plan
Elevate Medical will call to review your labs and discuss a treatment plan that will benefit you.
